New Service Helping Tackle Health Inequalities

Haringey GP Federation has launched a new service to support patients who are disproportionately affected by long term health conditions.

The project, which has been commissioned by North Central London Integrated Care Board. It is working with Haringey primary care networks to proactively identify and engage patients who are often more infrequent users of healthcare services, but at greatest risk of hypertension, chronic kidney disease and chronic obstructive pulmonary disease by offering a health check and health promotion.

Research shows that in Haringey, Turkish speaking patients in the three primary care networks in the east of the borough are more likely to experience long term health conditions than many other patient populations.

Meeting to discuss new service

For this reason, the first phase of the project is supporting patients from Turkish speaking communities before moving onto different population groups and different parts of the borough in later phases.

Our experienced team held the first of a planned programme of community events on Sunday 27th November at the Alevi Cultural Centre in Hornsey.  The relaxed, informal atmosphere provided an ideal setting to begin health education work and a lively Q&A session shared helpful information about the service.  Our team were able to encourage attendance for health checks which will aid the diagnosis of long-term conditions, as well as provide wider social prescribing and support.
